CD packaged in a tri-fold manila chipboard sleeve, to which the CD is attached by a clear rubber spindle. 

This is a remastered CD version of the original '[r=2273691' (Peterbilt #3), which was issued as a 12" vinyl EP in 1990.

According to the Dischord Records website, this is also Dischord 163.5, but this catalog number does not appear anywhere on the release itself.
